The majority of men don't think about the quality of their sperm until it's time to start a family. However, there's a lot men can do before then to improve the chances of a healthy pregnancy and baby down the line. In very rare cases, when a sperm count reaches zero ('azoospermia'), it might even be irreversible, and the chance of conception gone forever. Even for alternative fertility solutions such as IVF, a minimum count of healthy sperm is needed. Unfortunately, fertility testing for men is far from common and the topic comes with a lot of shame and stigma for many.

Male fertility is also a crucial indicator of overall health in men. Sadly, recent studies have shown that male fertility is strongly declining worldwide (50-60% over the past 40 years, https://pubmed.ncbi.nlm.nih.gov/28981654/), and this trend is expected to continue.

Every sixth couple has difficulties falling pregnant. Typically, we hear women be told to "get checked". However, of all infertility cases:

  • 1/3 are caused by male reproductive issues,

  • 1/3 by female reproductive problems, and

  • 1/3 by both male and female reproductive challenges or unknown factors

Male infertility can be caused by:

  • Lifestyle factors (i.e. smoking, drugs, alcohol, lack of exercise, stress, obesity, etc.)

  • Poor quality sperm (i.e. low sperm count, poor sperm motility, abnormal shapes)

  • Hormonal issues (i.e. in the testicles or pituitary gland which produces hormones)

  • Medicine and drugs (i.e. chemotherapy, anabolic steroids, anti-inflammatory drugs)

  • Ejaculation disorders (i.e. no semen release during intercourse)

  • Damaged testicles (i.e. infection, injury, cancer treatment, defects, undescended testes)

  • Sterilisation (i.e. vasectomy)

Infertile to Worry-Free

1st Sperm Analysis - My Identity Crisis

Growing up, I have always felt my central life purpose was to become a great father. However, when I did my first sperm analysis back in September 2022, I received a shocking result indicating that I will never be able to become a father due to infertility. It threw me into a deep identity crisis and depression.

Of course I was immediately seeking medical advice. Unfortunately, the doctors told me that there's not much I can do, and I likely have a problem in my level of sperm production. The most likely cause was that my testes were struggling to produce sperm in normal quantities. As a treatment option, testosterone therapy was suggested. To become a father in the future, I would need to freeze my sperm prior and rely on IVF, given, as I was told, the effect of the therapy would be that my body would stop producing sperm completely.

This message didn't help my mental state. I knew, I could never make the decision to start this therapy if it would eradicate my natural ability to become a father completely. I also didn't like the idea of IVF and was stubborn wanting to conceive a child naturally.

A Journey Against All Odds

Fortunately, instead of giving up, I decided to take matters in my own hands, ignore the doctors' pessimistic outlook on my fertility and researched possible interventions that would help me get to the minimum parameter levels.

 

To be honest, it didn't look great in the beginning, as most advice that was claimed to be the most effective (i.e. stop smoking, stop drinking, stop being overweight) didn't resonate with me given I've never been a smoker, stopped drinking 5+ years prior and with a BMI of 22 didn't fall into the obese category either.

Other studies I saw showed promising results, however, only ever improved parameters by one- or at best two-digit percentages. To get to the minimum fertility threshold of 15 million / mL sperm concentration, I however needed a four-digit percentage increase. Regardless, every morning when I hesitated to do my new routines and interventions, I simply asked myself: "Do you want to have kids or not?", and immediately jumped straight out of bed to get going.

2nd Sperm Analysis - Unbelievable Results

After 11 weeks and 40+ interventions across 9 different categories, I took another sperm test. My goal was to slowly, step by step, build up towards the magic number 15. Though, when I saw the results, I couldn't believe my eyes: My sperm concentration skyrocketed to 49 million / mL, an unbelievable 3,400% increase.

I regained a lot of confidence and was so happy that my dream of becoming a great father one day wasn't over at all. Despite the positiv result, I also knew I wasn't completely there, however. While my volume also sufficiently increased to 5.0 mL, my progressive motility increased to 36% only (just above minimum).

3rd Sperm Analysis - Worries Gone!

I further refined my interventions and kept on track aiming for an even better result in another sperm test in the future. 19 weeks later (30 weeks after the initial test), I provided another sample that would finally relieve me of my fertility worries: Sperm Concentration (61 million / mL, +4,300% increase total), Volume (5.5 mL, +25% increase total), and Progressive Motility (59%, +90% increase total) were all tracking well above minimum required levels.
